Health History and Physical Forms are required for all Kindergarten and Preschool students every year and

new students entering Holy Trinity School for the first time. Health History and Physical forms must have a physician's signature.

 

 

 

 

 

 

 

 

 

One Emergency School Closing form per family for all students in Preschool through Grade 8. 

 

One Consent and Release of Liability for Use of Minor Likeness (Media Release) form per family for all students in Preschool through Grade 8.

 

One Parent/Student Handbook Contract per family for all students in K-8

 

One Emergency Medical Authorization form for each student in Preschool through Grade 8
